The practice will be closed for the afternoon, from 12pm, on Tuesday 21st February 2012.

Prescriptions normally available for collection on this afternoon will be available the following morning.

We will be open as usual on Wednesday 22nd February 2012.

If you require emergency treatment during this afternoon, please call 0115 978 1231 to be put through to the out of hours emergency GP service.

HOW TO REGISTER


If you wish to register as a patient and you live within the practice boundary, you will need to visit the surgery to complete a form.  The receptionist will check your address against the practice boundary map and ask you to choose which GP you would prefer to register with.  If you have no preference one will be allocated to you. You do not have to continue to see this GP but we would recommend you see the same GP when you have settled in as this helps with the continuity of care. 

Within the first few weeks of registering you must make an appointment to see the practice nurse for a new patient check.  This is to give the GP some basic information about you and your health and is essential if you need to see a GP before we receive your notes from your previous practice.
